HOW WE MANAGE FORM SUBMISSIONS

Form data that is collected to fulfill user requests are managed through our Customer Relationship Management system solely for the purpose of fulfilling requests submitted through our website. 

MEDIA

If you upload images to our website, you should avoid uploading images with embedded location data (EXIF GPS) included. Uploaded images are only shared with the intended recipients as requested through the form.

Articles, including career opportunity listings, on this website may include embedded content (e.g. videos, images, articles, etc.). If you submit information via a form, the information and its metadata are retained indefinitely. For users who register, request or share information on our website (if any), we also store the personal information they provide within a secure Customer Relationship Management platform for Life Connection of Ohio’s use only. Users can email info@lifeconnection.org to request to modify or remove their personal data at any time. 

WHAT RIGHTS YOU HAVE OVER YOUR DATA

If you have submitted form data on this website, you can request to receive an exported file of the personal data we hold about you, including any data you have provided to us. You can also request that we erase any personal data we hold about you. This does not include any data we are obliged to keep for administrative, legal or security purposes.
